Have you ever been assigned a case manager during your hospital stay and wonder what they do? At Cullman Regional, our case managers try to find the best, most efficient, economical way of meeting our inpatients’ needs with the available resources.
In Honor of Case Management Week, we would like to recognize our Case Management Team. This team consists of nurses, social workers, discharge planners and utilization review case managers who work together collaboratively to meet complex inpatient needs, as well as make provisions for current and future needs of patients. Utilization Review case managers review and observe a patient’s status and make sure the patient is in the correct status per commercial insurance guidelines. The UR case managers are an asset in making sure we are billing the insurance companies correctly. Our case managers also work closely with social workers to obtain services for people who need long term care or home care after hospitalization. Our discharge planners coordinate services for patients to be discharged and provide patient education and post-care follow-up.
