Cullman Regional is now offering child vaccines (ages 5-11). See below for child vaccine information.

For ages 12 and up, vaccines are offered through the Cullman Regional Urgent Care Center. Vaccines are by appointment only.  You can schedule an appointment by filling out the form below.

Vaccine Clinic Hours

Hours are Mondays from 9 am – 1 pm and Fridays from 8am – 11am. No appointments are available Tuesday-Thursday, Saturday and Sunday. When you arrive, please come into the Urgent Care Center to check-in.

Child Vaccines (Ages 5-11 only)

If you would like to schedule a COVID-19 vaccination for your child, you may schedule an appointment at Cullman Regional Medical Group – North by calling 256-735-5075. Online appointments are not accepted for children’s vaccines. Appointments are available only on Fridays from 8am -12pm. All children must have a parent or legal guardian with them to receive their vaccination.

Who’s eligible?

Ages 5 and older are eligible to receive the COVID-19 vaccine in Alabama. The Pfizer vaccine is the only COVID-19 vaccine currently authorized for ages 5 and older. For more information from the Alabama Department of Public Health on COVID-19 vaccination, click here.

Tips for Scheduling Your Appointment Online

After you complete the below form, and press submit, you WILL receive an appointment confirmation. To ensure that you receive a confirmation:

  1. Complete all form fields.
  2. Enter a cell phone number (not a landline).
  3. Select “yes” and check mark all applicable fields.
IMPORTANT: If after hitting submit, you do not receive an appointment confirmation, please double check all your form fields are complete and correct, and press submit again. You will not receive the vaccine without an appointment confirmation.



Individuals must bring their insurance cards and a form of identification, wear a shirt that gives access to the upper arm and wear a mask. Following the vaccination, recipients will be required to remain in an observation area for 15 minutes to ensure they do not experience any adverse reaction.
