In order to help speed up the vaccine scheduling process, Cullman Regional has launched the online scheduling tool below to allow community members to self-schedule themselves for the vaccine. Appointment slots will open up 5 days prior to any available clinics based on Cullman Regional’s current vaccine allocation. Patients must have an appointment to receive a vaccine. Those currently eligible for vaccine in Alabama are healthcare providers, nursing home residents, law enforcement officers, firefighters, persons age 65 and older, and additional groups of critical workers.
Starting March 22, ADPH will extend eligibility for COVID-19 vaccinations to include people age 55 and older, people with intellectual and developmental disabilities, and the population defined as Allocation Phase 1c in the Alabama COVID‐19 Vaccination Allocation Plan which includes people age 16 to 64 with high-risk medical conditions. Tips for Scheduling Your Appointment Online
After you complete the below form, and press submit, you WILL receive an appointment confirmation. To ensure that you receive a confirmation:
- Complete all form fields.
- Enter a cell phone number (not a landline).
- Select “yes” and check mark all applicable fields.
IMPORTANT: If after hitting submit, you do not receive an appointment confirmation, please double check all your form fields are complete and correct, and press submit again. You will not receive the vaccine without an appointment confirmation.
Individuals must bring their insurance cards and a form of identification, wear a shirt that gives access to the upper arm and wear a mask. Following the vaccination, recipients will be required to remain in an observation area for 15 minutes to ensure they do not experience any adverse reaction.
