The Cullman Regional COVID vaccine clinic is located on the hospital’s Mobile Unit in the parking area of Professional Office Building (POB) 3 on the east side of campus. This location is most easily accessed through the hospital’s main entrance at the traffic light off Highway 157. Vaccines are by appointment only.  You can schedule an appointment by filling out the form below.

If no appointments are available, please check back the following day. We post new appointment times regularly.

Vaccine Clinic Hours

Hours are Monday, Tuesday, Wednesday & Friday from 9 am – 1 pm. No Thursday appointments are available. When you arrive, please pull up to the mobile unit and a staff member will give you further instructions.

Who’s eligible?

Ages 12 and older are eligible to receive the COVID-19 vaccine in Alabama. The Pfizer vaccine is the only COVID-19 vaccine currently authorized for ages 12 and older. For more information from the Alabama Department of Public Health on vaccine planning and prioritization, click here.

Tips for Scheduling Your Appointment Online

After you complete the below form, and press submit, you WILL receive an appointment confirmation. To ensure that you receive a confirmation:

  1. Complete all form fields.
  2. Enter a cell phone number (not a landline).
  3. Select “yes” and check mark all applicable fields.
IMPORTANT: If after hitting submit, you do not receive an appointment confirmation, please double check all your form fields are complete and correct, and press submit again. You will not receive the vaccine without an appointment confirmation.



Individuals must bring their insurance cards and a form of identification, wear a shirt that gives access to the upper arm and wear a mask. Following the vaccination, recipients will be required to remain in an observation area for 15 minutes to ensure they do not experience any adverse reaction.

Also, if possible, please download, print, complete and bring the vaccine consent form with you to your appointment.
