Medical Records
Cullman Regional’s Health Information Services Department is responsible for maintaining and managing accurate medical records for patients who receive care through Cullman Regional, Cullman Regional Medical Group and Hartselle Health Park. It is important for you to be informed about your health and the healthcare you receive so we offer several ways for you to obtain and view your medical record.

Options to receive a paper copy of your medical record:
- Visit Cullman Regional Medical Records Department located on the second floor of Professional Office Building I in suite 260. You will be asked to complete a records request form and show photo identification.
- Download and complete the records request form at home then bring it and your photo ID to the Medical Records Department.
- Download and complete the records request form at home then mail it to: Cullman Regional Medical Records Department, PO Box 1108, Cullman, AL 35056 (NOTE: If you are mailing the records request form, your signature must include a witness signature.
- Use an online request form through Cullman Regional’s Health Information partner HealthMark Group to have your Medical Records emailed to you at no charge.
View and download medical record through Cullman Regional Patient Portal:
You can also view medical records on the Cullman Regional Patient Portal. If you have not already created an account, you can request patient portal access by calling 256-737-2500. Medical Records staff will authorize the system to send an email with instructions on how to activate a patient portal account. The email message will come from Cullman Regional.
If the patient is 14 or older, they must sign the medical records request form to obtain a copy of their medical record.
Other services
Birth Certificates
Before you leave the hospital, you will be given a document that shows a record of the birth. This is not an official birth certificate and cannot be used for legal purposes. You will also be given an application to order an official certified copy of your child’s birth certificate from the State. You may also obtain a certified copy of a birth certificate at any county health department in Alabama or go online to Alabama Department of Public Health for information on how to request a certified birth certificate from the State after you leave the hospital.
Listing Father on Birth Certificate
If the child’s parents are unmarried, the father can only be listed on the birth certificate if he acknowledges the paternity of the child. To acknowledge paternity, the Acknowledgement of Paternity Form must be completed and signed by both parents in the presence of a notary. The hospital birth certificate representative can provide the form and notary services. Each parent must show a valid photo ID (driver’s license, school ID, passport, military ID, state/government ID) during business hours.
If you do not wish to acknowledge paternity during your hospital stay, an Acknowledgement of Paternity can be submitted any time prior to the child’s 19th birthday by contacting the Department of Human Resources county office or the Center for Health Statistics, P.O. Box 5625, Montgomery, Alabama 36103-5625.
Death Certificates
You may go to any county health department in the State of Alabama to obtain a certified copy of the Alabama death certificate. You may also visit the Alabama Department of Public Health website and download a mail-in application.
